Your Kyndred Care Journey 

A Dedicated Hour to Start Your Journey. Your Kyndred care journey starts from the comfort of your home and is established through a 60-minute online intake appointment with one of our virtual care providers (NP, PA, MDs). We’ll spend the time taking a thorough social, medical, family, and lifestyle history that allows us to deliver on our comprehensive care model, and of course answer any questions you have.

Meet Your Care Advocate. You’ll then be paired with your care advocate, who will help you coordinate your care needs with Kyndred and help navigate the services required to address your health and wellness goals.

A Personalized Care Plan for Your Journey. Now with your personalized Kyndred care plan in hand, you’ll have a clear roadmap tailored to your unique goals. As a living document, your care plan will evolve with you, ensuring you and your care team are always on the same page.
